Are Supplements Safe? What to be aware of

As an internal medicine physician, I frequently encounter patients burdened with extensive medication lists. The rise of online purchasing has made accessing health supplements convenient. However, potential interactions between prescribed medications and supplements remain a concern.

I vividly recall a patient who presented with a staggering 80 supplements to review! This extreme case highlights the growing trend of supplement preference over traditional medications. I stress that supplements must be approached cautiously, like medications, as they can have side effects and interactions. The medical principle of “do no harm” extends to supplements

Here are notable interactions, side effects, and situations to be aware of:

  1. Turmeric and fish oil: While used for inflammation, these supplements can also thin the blood. Caution is necessary if taking blood thinners.
  2. Statins: These cholesterol-lowering drugs can deplete Coenzyme Q10, so supplementing with it is often advisable.
  3. Garlic, Gingko biloba, Vitamin E: These cardiovascular supplements can increase bleeding risk.
  4. Liver effects: Supplements like kava, black cohosh, and aloe vera can impact liver function. These are well known. But many other supplements can affect the liver as well. For example, Valerian root, a common and great supplement for sleep and anxiety, may affect the liver and should be monitored.
  5. Pregnancy and breastfeeding: Many supplements aren’t recommended during these times.
  6. Chemotherapy: Consult an oncologist before any supplement use.
  7. Duration: There is limited data available regarding the duration of use for certain supplements.

For any supplement you consider taking, it is vital to understand the purpose behind it and be aware of potential interactions and side effects. If you have questions or concerns, collaborate with your functional medicine physician for guidance.
