At SouthPark Functional Medicine, we believe that the most effective healthcare is built on a deep, consistent, and direct partnership between a patient and their physician. Our fee structure is designed to support this singular focus.
Your investment covers Dr. Mahtani’s extensive time and expertise, both during and between your appointments. Unlike platform-based models with multiple providers, your entire case is personally managed by Dr. Mahtani from start to finish. This includes in-depth analysis, personalized research, and the creation of a sophisticated health plan designed only for you.
Our practice operates on a fee-for-service basis and does not participate in any insurance plans. This allows us to provide a level of care that is not possible within the constraints of the traditional insurance model. Care at SPFM is structured into two distinct phases: The Diagnostic Phase and Ongoing Care.
PHASE 1: The Diagnostic & Foundational Phase
Your journey begins with a comprehensive evaluation to uncover root causes and build your initial protocol.
- Comprehensive Initial Consultation — $750
- Expert Pre-Visit Analysis: Before you meet, Dr. Mahtani personally conducts an extensive review of your entire medical history, previous labs, and all intake forms. This ensures your time together is dedicated to high-level discussion and strategy.
- Dedicated Physician Time (60 minutes): You will spend a full hour, one-on-one, with Dr. Sarita Mahtani, MD, IFMCP. This unhurried visit is focused on understanding your full story and co-creating the initial steps of your health journey.
- A Foundational Action Plan: You will leave with a clear, personalized plan that includes orders for conventional or advanced functional lab testing, initial dietary and lifestyle modifications, and targeted supplement recommendations.
- A Comprehensive Summary & Roadmap: Following your visit, you will receive a detailed electronic report outlining your initial treatment plan.
- Direct Portal Access During Your Diagnostic Phase: From your initial consultation through your Lab Review visit, you’ll have direct portal access to Dr. Mahtani for questions as your labs and initial protocol take shape.
- Comprehensive Lab Review & Follow-Up — $375
- Once your initial functional labs are complete, your second visit is dedicated to thoroughly analyzing your new clinical data. Based on these findings, Dr. Mahtani will refine and finalize your targeted health protocol.
PHASE 2: Continuing Your Care: The Membership Partnership
Functional medicine works best as an ongoing partnership rather than a series of isolated visits. After your initial consultation and first follow-up, patients are invited to continue their care through our membership — a simple, predictable way to stay closely connected with Dr. Mahtani as your health plan evolves.
- The SPFM Membership — $2,950 per year
Includes over $1,100 in consultations at standard rates, plus year-round direct access to Dr. Mahtani.- Priority scheduling: members receive first access to Dr. Mahtani’s calendar for follow-up visits.
- Three follow-up consultations per year, approximately every four months (included).
- Direct messaging with Dr. Mahtani through our secure patient portal, for questions related to your active treatment plan. Messages are answered within 1–2 business days.
- Ongoing review and refinement of your personalized plan as new results and information arrive.
- Membership is billed annually, or in two payments of $1,475 every six months, and renews on your enrollment date.
Membership is optional and is offered following your first follow-up visit, once your foundational plan is in place. For patients whose health is stable and who prefer to continue their care on an as-needed basis, follow-up consultations are available à la carte ($375/visit).
(Please note: Following the Diagnostic Phase, direct portal messaging is reserved exclusively for members, so Dr. Mahtani can dedicate protected time to complex medical analysis. À la carte patients with a clinical question are warmly invited to schedule a follow-up consultation, which allows Dr. Mahtani to review your chart and give your question the attention it deserves.)
Insurance & Reimbursement
Upon request, we are happy to provide you with a detailed superbill containing the necessary medical codes for you to submit to your insurance company. Many patients are able to receive partial reimbursement for their consultation if their plan includes out-of-network benefits.
